The Greater Manchester and Eastern Cheshire Strategic Clinical Network are currently developing a Palliative and End of Life Care Framework. The Framework will set a number of objectives and outline the outcomes that the people in Greater Manchester can expect, so to live well during the last year of life with dignity, in the place of their choice, regardless of disease, setting, age and circumstance.
The Framework will contain 12 individual focused commitments and 4 system enabling commitments which have been developed in collaboration with members of the emerging Greater Manchester Palliative and End of Life Care Board, who have brought a wealth of experience from health and social care commissioning, service provision, and those that have personally experienced the death of a loved one.
Now that the commitments have been developed, it is important that they are shared further as part of a wider consultation process and the Greater Manchester Health and Social Care Partnership would really value your feedback before the commitments are presented to the Board as a final draft. They would be extremely grateful if you could set aside approximately 20 minutes to complete an online survey so a Framework can be created that will significantly enhance the quality of care for people in the last year of life and and those closest to them.
The online survey can be accessed here and is open from Monday 14 January to Thursday 31 January 2019. Please feel free to forward this information to your networks, friends, family and colleagues and encourage them to take part.
The draft Greater Manchester commitments to individuals with Palliative Care needs approaching or within the Last Year of Life, can be viewed here should you wish to read prior to beginning the online consultation process.
